The Physio Josh is a private home-based physiotherapy service operated by Josh Ellis, a HCPC registered physiotherapist. I am the data controller for any personal information collected through this website and during the course of treatment.
When you use this website or engage my services, I may collect the following information:
Through the booking form: your name, email address, phone number, the service you're interested in, and a description of your condition or symptoms.
During assessment and treatment: relevant medical history, details of your condition, treatment notes, outcome measures, and any other clinically relevant information needed to provide safe and effective care.
Automatically: this website does not use cookies or tracking scripts. No analytics data is collected.
I collect and process your personal data for the following purposes:
To respond to your enquiry — when you submit the booking form, I use your contact details to get back to you and arrange an appointment.
To provide physiotherapy treatment — clinical notes and medical history are essential for safe, effective, and personalised care. I am required by the HCPC to maintain accurate patient records.
To meet legal and professional obligations — as a regulated healthcare professional, I am required to keep clinical records and may need to share information with other healthcare providers involved in your care (with your consent).
I process your personal data under the following legal bases as defined by UK GDPR:
Consent — when you submit the booking form, you consent to me using your details to respond to your enquiry.
Legitimate interest — to provide and improve my physiotherapy services.
Legal obligation — to comply with HCPC registration requirements and other applicable regulations.
Vital interests / healthcare provision — clinical data is processed under Article 9(2)(h) of UK GDPR as necessary for the provision of health care.
Your personal information is stored securely. Booking form submissions are received via email and are not stored on this website's server. Clinical notes are stored securely in accordance with HCPC standards and CSP guidance on record keeping.
I take appropriate technical and organisational measures to protect your data against unauthorised access, loss, or misuse. Electronic records are password-protected, and paper records (if any) are stored in a locked cabinet.
Booking enquiries: if you do not proceed with treatment, your enquiry details are deleted within 12 months.
Clinical records: in line with CSP and NHS guidance, clinical records are retained for 8 years after the last date of treatment (or until a child patient turns 25, whichever is longer). After this period, records are securely destroyed.
I will never sell your personal information. I may share your data in the following circumstances:
Other healthcare professionals — with your explicit consent, I may share clinical information with your GP, consultant, or other healthcare providers involved in your care.
Insurance providers — if you are claiming through health insurance, I may provide necessary clinical information to your insurer with your consent.
Legal requirements — if required by law, court order, or regulatory body.

If you are unhappy with how I have handled your data, you have the right to lodge a complaint with the Information Commissioner's Office (ICO) at ico.org.uk or by calling 0303 123 1113.
Booking requests submitted through this website are requests only and are not confirmed until you receive written confirmation (via email or text) from me. I will aim to confirm or arrange your appointment within 24 hours of receiving your request.
I ask for at least 24 hours' notice if you need to cancel or reschedule an appointment. Cancellations made with less than 24 hours' notice may be charged at the full session rate. I understand that emergencies happen — please just let me know as soon as possible.
All prices are as listed on the Services page and include travel within my core service area. A travel supplement may apply for addresses outside this area — I will always discuss this with you before your appointment. Prices may be updated from time to time; the price confirmed at the time of booking is the price you pay.
Payment is due at the end of each session unless otherwise agreed (e.g. for block bookings, which are paid in advance). I accept bank transfer, card payment, and cash. Receipts are provided for all payments and can be used for insurance claims.
Before any treatment begins, I will explain what I plan to do, why, and any risks involved. Treatment will only proceed with your informed consent. You have the right to withdraw consent and stop treatment at any time.
I am registered with the Health and Care Professions Council (HCPC), a member of the Chartered Society of Physiotherapy (CSP), and hold full professional indemnity and public liability insurance. I adhere to the HCPC Standards of Conduct, Performance and Ethics and engage in ongoing professional development.
I take complaints seriously and aim to resolve any concerns promptly. If you are unhappy with any aspect of my service, please contact me directly in the first instance. If we are unable to resolve the issue, you can escalate your complaint to the HCPC at hcpc-uk.org.
